It is a known issue that the oil pump nut can loosen itself and come off, which causes an immediate loss in oil pressure and will most likely spin the bearings, if not cause additional damage. It seems like you're at a higher risk for it if you spin the car without getting the clutch in in time, but there really isn't any rhyme or reason to it. People either tack the nut on with a welder or you can drill a hole in it and safety wire it to the gear.
I don't trust myself to put a tiny hole in the corner of a hardened steel nut without a drill press so I'm gonna use one of the "kits" they sell, which Christina luckily has a spare for, so I dont even have to wait for it to come in the mail.
