I'm going to venture a guess that he had minimal coverage on the bike. I don't carry very good bike insurance, myself... but that's probably for different reasons.
To answer Paul's question, I believe his health insurance did/would have covered his medical costs. Health insurance doesn't offer non-income compensation for patients during recovery, though. He wasn't able to work while he was healing, hence, he had losses.
